Why your doctor probably doesn't know
Cannabis has been federally illegal for decades, which means most medical schools taught very little about it and most physicians learned even less. When a patient asks about cannabis interactions, many doctors cannot answer confidently—not because they are unprepared, but because the field itself was sidelined.
Now that cannabis is legal in most states, patients are using it while taking prescription medications, and the interactions are real. Over 407 documented drug interactions exist between cannabis and common medications. This is not a matter of folklore or caution; it is pharmacology.
The good news is that most interactions are manageable. You do not have to choose between cannabis and your medication. You do have to know what you are combining and monitor yourself for signs that the combination is not working.
How cannabis changes how your body processes medications
Cannabis is metabolized primarily by the liver—specifically by enzymes called CYP3A4 and CYP2C9. These same enzymes metabolize hundreds of common medications. When cannabis and another drug both need the same enzyme, they compete. The one that wins metabolizes faster; the one that loses stays in your bloodstream longer and builds to higher concentrations.
This is called enzyme inhibition, and it is the source of most interactions. Cannabis inhibits CYP3A4 and CYP2C9, which means it can slow the breakdown of medications that depend on these pathways. The result is higher blood levels of the drug, potentially higher effects, and potentially higher side effects.
Some interactions work the other way—cannabis can also be a substrate for these enzymes, meaning medications can affect how quickly cannabis is cleared from your body. Either direction changes the experience.
The highest-risk medication categories
Blood thinners (warfarin, apixaban, rivaroxaban): Cannabis can increase bleeding risk. If you take these, discuss cannabis use with your cardiologist. Dosing may need adjustment, and you may need more frequent INR checks.
Sedatives and sleep aids (benzodiazepines, zolpidem): The combination amplifies drowsiness and impairs judgment. Using both multiplies the risk of falls, accidents, and respiratory depression.
Antiarrhythmics (medications for heart rhythm): Cannabis raises heart rate and blood pressure acutely. Combined with medications that alter your electrical conduction, the result can be unpredictable.
Immunosuppressants (for transplant patients, autoimmune conditions): Cannabis may reduce their effectiveness, compromising the condition they are meant to control.
Antiepileptics (phenytoin, valproate, clobazam): Cannabis and these medications compete for the same metabolic pathway. Levels of either can rise unexpectedly.
Antidepressants and antipsychotics: Cannabis can worsen psychotic symptoms and may interact with SSRIs to increase serotonin effects. People with a family history of psychosis should avoid THC entirely.
Lower-risk but still worth knowing
Pain medications (NSAIDs, opioids): Cannabis can increase sedation with opioids but does not usually change NSAID clearance dramatically. Still, combining CNS depressants (cannabis + opioids) requires caution.
Blood pressure medications (ACE inhibitors, beta blockers): Cannabis can lower blood pressure acutely, and the combination might push you too far. Monitor your BP if you use both.
Diabetes medications: Cannabis may affect blood sugar. Patients on insulin or sulfonylureas should monitor glucose more closely when adding cannabis.
Statins and other lipid-lowering agents: Moderate interaction risk. Cannabis inhibits some statin metabolism, potentially raising blood levels.
THC versus CBD—different interaction profiles
THC and CBD are metabolized differently. THC is the primary psychoactive and the one that inhibits CYP enzymes most strongly. CBD also inhibits these enzymes but through a different mechanism and at different affinity levels.
High-THC cannabis is more likely to cause interaction effects. High-CBD or balanced products may be safer for people on medications. This is not a universal rule—it depends on the specific medication—but it is a useful first consideration.
CBD-only products (isolates, broad-spectrum) carry lower interaction risk than full-spectrum flower, especially when THC is absent.
What to do if you take medications
Do not assume your doctor knows cannabis pharmacology. Many do not, and that is not a criticism—it reflects the state of medical education until very recently.
Bring a list: Write down every medication, supplement, and herbal product you take. Bring it to your doctor and ask specifically about cannabis interactions. If they cannot answer, ask for a referral to a pharmacist who specializes in cannabis.
Start low: If your doctor clears you to use cannabis, begin with a low THC dose (5mg or less) and increase gradually. Watch for changes in medication effectiveness or side effects.
Monitor symptoms: Keep a log of your cannabis use (date, dose, product type) and any changes in how you feel or how your medications are working. Bring this to your next appointment.
Time your doses: Some interactions can be minimized by spacing cannabis and medications apart. If possible, do not take them at exactly the same time.
Consider CBD-dominant products: If you need the medical benefits of cannabis but take multiple medications, ask about high-CBD, low-THC products or CBD isolate.
Red flags that demand immediate attention
Unexpected bleeding or bruising (especially if on blood thinners): Call your doctor.
Excessive sedation, dizziness, or confusion: Do not drive. This could mean medication levels are too high.
Changes in heart rhythm (palpitations, chest pain, shortness of breath): Seek medical attention.
Changes in blood sugar (unusual thirst, frequent urination if diabetic): Test and call your provider.
Worsening mental health symptoms (increased anxiety, paranoia, hallucinations): Stop cannabis and call your doctor immediately.
Sudden changes in medication effectiveness: Your medication may not be clearing properly. Contact your doctor.
The conversation to have with your provider
Bring a product: Show your doctor or pharmacist the label from the cannabis product you want to use. They need to know the THC:CBD ratio and whether it is full-spectrum or isolate.
Be honest: Tell them how often you plan to use it and what dose. This lets them model the interaction accurately.
Ask for specifics: Instead of "Is cannabis okay?", ask "Does cannabis interact with [medication name]? Should I adjust my dose? Do I need more monitoring?"
Plan monitoring: If your doctor clears you, agree on how often to check in and what symptoms to watch for.
Get it in writing: Ask your pharmacist to document the interaction check in your record. This protects you and your doctor.
